Despite the theoretical elegance, extensive clinical lore, and case study reports on the effectiveness of Ego State Therapy (EST) for PTSD, EST has not undergone controlled studies of its effects meeting evidence based criteria. On the basis of Ego State (Watkins & Watkins, 1997) and abreactive ego state hypnosis theory (Barabasz, Barabasz & Watkins, 2011; Watkins & Barabasz, 2008, Chapt. 10), thirty participants meeting DSM-IV-TR criteria for PTSD were assigned, in balanced order, to either abreactive ego state therapy (EST) or to a control condition derived from U.S. Veterans Administration active treatment procedure. The EST condition followed a manualized protocol (Barabasz, Christensen & Watkins, 2010) involving a single cotherapy session of 5-6 hours. Efficacy was measured pre, post, and at one month follow-up using the Davidson Trauma Scale (DTS), the Beck Depression Inventory-II (BDI-II), and the Beck Anxiety Inventory (BAI). Two way analyses of variance (2 groups X 4 measurement periods), Mann-Whitney U, and Wilcoxon Signed-Ranks tests, appropriate to the distributions of data, showed statistically significant positive effects of the EST treatment for pre-treatment in contrast to all three post-treatment periods on all measures, as well as, the significant superiority of the EST treatment versus the active control condition.
