Abstract
Canine prognostic factors radiation therapy subcutaneous and intramuscular hemangiosarcoma Chemotherapy
Hemangiosarcoma is a highly aggressive and malignant tumor derived from the endothelial cell line, characterized by early and widespread metastasis. While surgical excision remains the primary treatment modality for subcutaneous and intramuscular hemangiosarcoma, its infiltrative nature and propensity for recurrence often necessitates the use of adjuvant therapies, such as radiation therapy, to improve local control and overall survival outcomes. The purpose of this study was to assess outcome and prognostic factors for dogs with subcutaneous and intramuscular hemangiosarcoma treated with radiation therapy and doxorubicin. Clinical data was collected retrospectively between 2005 to 2023 and included 55 dogs. The overall response rate to radiation therapy in a macroscopic disease setting was 81%. Loco-regional recurrence occurred in 20% of dogs at a median time of 306 days after radiation therapy. The presence of anemia (P = 0.0017), metastasis (P = 0.0001), high mitotic score (P = 0.028), and lack of neoadjuvant surgery (P = 0.013) were found to predict loco-regional recurrence. The overall median survival time for all dogs was 306 days. The presence of anemia (P = 0.001), metastasis (P = 0.013), lack of neoadjuvant surgery (P = 0.013), and tumor size (0.002) were found to be a negative prognostic indicators for survival. There was no significant difference in survival time between dogs with respect to hemangiosarcoma location or the use of chemotherapy. Radiation therapy and concurrent doxorubicin was well tolerated with minimal side effects. The prognosis for subcutaneous and intramuscular hemangiosarcoma remains relatively poor. A greater chance of long-term survival might result from adequate local control with surgery and radiation therapy for smaller tumors without concomitant anemia or metastases.
