This review of literature analyzes six research studies which investigated the relationship between home health care and hospital readmission rates for patients with congestive heart failure (CHF). Four of the research studies found that the use of home health care could reduce hospital admissions and save money. One study found that is was more costly to utilize home health care and another study showed that there was no significant difference. This information was utilized to develop a proposed program specifically targeted to patients with CHF. This program empowers nurses and advanced practice nurses to provide preventative care that improves the quality of the patient's life and is fiscally responsible to the healthcare industry.
