Hospitals -- Admission and discharge Heart failure Nursing
More than five million Americans are living with heart failure (HF), a chronic condition which impairs the heart's ability to supply sufficient blood to meet the body's oxygenation requirements (National Heart Lung and Blood Institute, 2010). There are more than one million HF related hospitalizations annually with a 1 in 4 readmission rate within 30 days of hospitalization (Joynt, 2011). There are a variety of techniques currently in practice in an attempt to decrease HF readmissions and therefore reduce the financial and clinical burden on the healthcare system. The purpose of this paper is to identify current strategies that reduce HF hospital readmissions and recommend key components for nurse practitioners to help reduce HF readmissions.
